Medical Form
All campers (Youth, Adults, Parent Helpers, OOS, etc), must provide a Medical Form. This can be the Physical Fitness form (linked here) a print out from Myscout, or a form from your home organization.

Please put your whole group in an envelope with your Group Name/Number, Subcamp & Contact Scouters (at camp) contact information on the front of the envelope. 
​ Adventure Application Forms (AAF's)
Scouts Canada Troops are required to provide a copy of their AAF. This can be from Scouts Tracker or the form linked here. AAF's must be approved by your GC or SRM.

Patrol List
All groups are required to provide a list of who is in each patrol. The patrol how your groups plans to divide up while going around to do the program/event stations. They can be as big as the whole group or as small as you wish (usually 6-8 youth).
